Free assistance with

Prescription Medication​

We look for the best cost of a medication for our Clients. Our goal is to get it for a lower cost or for free.  This can be done electronically by fax or email. We also can use the US mail service. We will see clients in the office if that is what the client prefers.

Items you will need to provide us:
1. Copies of Driver’s license or picture ID
2. Pharmacy print out showing the out of pocket expenses paid for the current year to date.
3. A list with each medication, strengths and frequency.
4. Each doctor’s full name, address, phone and fax numbers.
  
Each Prescription Assistance program has different requirements for approval; patients may be asked for additional information.

Line should be answered during Heart USA business hours. If calling outside of business hours, leave a voicemail, and your call will be returned during business hours.


Medical Transportation Program Operating Hours:
Tuesday, Wednesday, Thursday only
Medical appointment times from 11 a.m. -2 p.m. only


Patient Qualifications:

Patients needing transportation for medical appointments only.
Patients must be able-bodied and not require assistance with walking. Program is unable to accommodate wheelchairs.
50 mile radius preferred (both for pick up/drop off location, and for the medical appointment)
Patients must be a CHW Program client of Heart USA (if patient is not a current CHW Client, the patient will be enrolled when they call to schedule transportation)
Patients must not qualify for any other transportation benefits (ex: veteran, Medicaid)

Medical Supplies

We accept donations of gently used medical supplies at our office.  These items are donated to individuals on a first come first serve basis. 

Community Health Workers

Our experienced and highly trained CHW’s provide the highest quality of care to both patients and their families. Our goal is to help individuals through home visits by addressing needs within the family and suggest ways to improve their lives. 
 Heart USA has been in Western Kentucky for over 16 years.  We have touched the lives of 1 out of every 3 people in the area. 
We are available at a number of health expos and community benefits throughout the year.
